New River State Park would like to document each hellbender sighting. If you locate one, please let them know as much of the below information as you can and a photo would be great!

1. Size from nose to tip of tail or from nose to cloaca vent?
2. Location description, X on a map or GPS coordinates?
3. How deep was the water?
4. Was it located under a rock, along rocky bottom, among aquatic vegetation or on flat sandy bottom or muddy bottom?
5. Was the water moving quickly or was it still and calm ?
6. How did you come by the critter (fishing, canoeing, swimming, driving by,)
7. Date and time of day spotted?
8. Temperature, warm or cold day?
